| If your school was selected to compete in the Ninth Annual ProStart Culinary Team Competitions, all of your paperwork is due on Friday, January 9, 2009.
Please refer to the Required Paperwork Checklist which will assist you in determining the requirements for your paperwork submission.
If we do not receive ALL required paperwork by 5:00 pm on January 9, 2009, your team will be replaced by the first team on the waiting list and your school will not be allowed to compete in the 2010 competition.
If you are currently in the competition and decide not to compete, please let us know ASAP so a team on the waiting list can take your place. If you back out of the competition prior to January 9, 2009, you will not be penalized.

Required Paperwork:
- School Entry Form
- Participant Waiver and Authorization and Release Form (one per competing student)
- We do not need this form for students that are not competing in the competition.
- Proof of Age (one per competing student)
- A copy of the student’s driver’s license, birth certificate or high school identification indicating age and year in high school will be acceptable documentation of Proof of Age.
- We do not need this form for students that are not competing in the competition.
- Gourmet Meal Recipes
- We just need a copy of the recipes. We do not need the food costing information.
